Why soft tissue work helps after ACL surgery

After surgery, your body protects the knee. Muscles around it can tighten up and stay guarded, the quad can be slow to fire, and scar tissue forms around the incisions and graft site. Compensating with the other leg can cause tightness in the hips and back too.

Massage can help ease that tension, improve how the tissue around the knee moves and make your PT exercises feel more comfortable. Many athletes find it helps them hit their range-of-motion goals and feel more confident as they progress.

What we work on

What we do depends on where you are in recovery and what your surgeon and physical therapist have cleared.

  • Quads and hamstrings that have tightened up
  • Calves and the back of the knee
  • Scar tissue around incisions, once fully healed and cleared
  • Hips and glutes that work overtime while you compensate
  • The opposite leg and low back
  • Gentle movement within your allowed range

Timing and clearance

Always check with your surgeon or physical therapist before starting massage after surgery. Early on, work stays away from the incisions and focuses on surrounding muscles and the rest of your body. Direct scar tissue work only starts once incisions are fully closed and you've been cleared.

Let us know what phase you're in and what your PT is focused on. We're happy to coordinate so our work supports theirs. As you move toward running, cutting and return to sport, sessions become more like sports massage, keeping the whole leg healthy under the new load.

Between sessions

Your physical therapy program is the foundation of ACL recovery. Massage works best when you're consistent with it. A few things that help:

  • Do your home exercise program every day, even when you don't feel like it
  • Keep working on knee extension and quad activation, as your PT directs
  • Keep the rest of your body moving to avoid stiffness from compensating
  • Prioritize sleep and nutrition, which drive tissue healing
  • Track your range-of-motion milestones so we can match our work to your progress
  • Follow your surgeon's timeline for running and return to sport

Other knee surgeries

The same approach applies to meniscus repairs, other ligament reconstructions and many other knee procedures. Bring us your surgeon's or PT's guidance and we'll build sessions around it.
